Insurance & Fees
Clear, upfront information about what your care will cost — because you deserve to know before your first appointment.
At Clarivolve, we believe transparency about cost is part of good clinical care. Whether you're using insurance or paying out of pocket, you should know what to expect financially before your first visit — not after.
Accepted Insurance Plans
I'm currently in-network with the following insurance plans through Alma and Headway:
Aetna
Cigna
United Healthcare
Anthem Blue Cross
Optum
Quest Behavioral Health
How insurance works here:
Your out-of-pocket cost depends on your specific plan — copay, deductible, and coinsurance all vary. Before your first appointment, I'll verify your benefits so you know exactly what to expect. Most patients with in-network coverage pay a copay ranging from $20 to $60 per visit, though high-deductible plans may result in higher out-of-pocket costs until the deductible is met.
If your insurance isn't listed above, you may still be able to use out-of-network benefits — see the section below.
Self-Pay Options
Many patients choose to pay out-of-pocket — either because they don't have coverage with one of my in-network plans, because they prefer privacy (insurance requires a diagnosis on record), or because their deductible makes cash-pay effectively the same cost.
Initial Psychiatric Evaluation — $250
60-minute comprehensive appointment. Includes full psychiatric history, symptom review, diagnosis, and initial treatment plan.
Follow-Up Medication Management — $100
20-30 minute appointment. Used for medication adjustments, monitoring, and ongoing care. Frequency based on clinical need.
Comprehensive ADHD Evaluation — $550
Includes psychiatric intake, QbTest computerized assessment, clinical review, written summary, and results appointment. [Learn more →]
Why I Accept Both Insurance and Cash-Pay
I chose to be in-network with major insurance plans because I believe quality psychiatric care shouldn't be reserved for those who can afford to pay fully out-of-pocket. At the same time, I keep cash-pay rates transparent and reasonable because some patients — those with high-deductible plans, those who value privacy, or those whose insurance I don't accept — deserve a clear alternative.
Whichever path you choose, the quality of care is the same.
Good Faith Estimate (No Surprises Act): Under the federal No Surprises Act, patients who are uninsured or who choose not to use insurance have the right to receive a Good Faith Estimate of expected charges before scheduled services. Clarivolve Mental Health will provide a written Good Faith Estimate prior to your initial appointment. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the charge. For questions or to request an estimate, please contact us at (951) 517-3701 or anikpour@clarivolve.com. For more information, visit www.cms.gov/nosurprises.
Frequently Asked Questions
-
Not at this time. If you have Medicare or Medi-Cal and need psychiatric care, I can help point you toward appropriate in-network providers.
-
Yes. All psychiatric services qualify as eligible medical expenses.
-
The clinical intake portion can typically be billed to insurance. The QbTest computerized testing component is generally not covered by insurance, which is why the comprehensive evaluation is offered as a cash-pay bundle. I'll explain the options during your consultation call.
-
For many patients with high-deductible plans, cash-pay rates end up being similar or even lower than what they'd pay toward their deductible. If this is your situation, I'll help you think through which option makes more sense financially.
-
I don't currently offer sliding scale, but I do accept major insurance plans which makes care accessible for most patients. If cost is a barrier and my in-network options don't work for you, I'll help connect you with appropriate resources.
-
Once you submit a consultation request, I or a member of my billing team (through Alma/Headway) will verify your benefits and share the expected costs before your first appointment. You'll never be surprised by a bill.
